Our Beloved Mitchell,
In Remembrance of Our Beloved Mitchell D. Polk “Unique” Sunrise Sept. 4, 1961 Sunset Aug. 23, 2016 Mitchell Polk was born on Sept. 4, 1961 to the parentage of the late June Polk in Forney,TX. He was 3rd of 6 children. He received his formal education at James Madison High School in Dallas,TX. Mitchell was also an active member at CornerStone Baptist Church. He leaves to cherish his memories with 3 sisters Cynthia Miller who preceded him in death, Diane Polk and Jacqueline Polk, 2 brothers Michael Polk and Bruce Polk, 5 neices Arshene Miller, Shatonga Miller, Lekecia Paul, Meschell Grady, and Carla Polk, 4 nephews Brandon Miller, Calvin Overstreet, Kevin Fancher, Kyle Fancher and a host of family and friends…

I remember when we first met, you was extremely happy and filled with so much joy. Throughout the small amount of time we have known one another, we have established the best relationship a uncle and niece could have. Rest up for now, You deserved your wings fly high! You are loved and Greatly missed, and you will always be apart of my heart..
